I, the custodial parent / legal guardian, give my permission to my son/daughter to participate in any and all OPFY activities during the current season, Including post-season completion, I am aware that football is a full contact sport and cheerleading requires strenuous, physcial activities. I assume the entire risk ad hazards incidental to such participation including transportation to and from activities and do hereby WAIVE, RELEASE, ABSOLVE, INDEMNIFY, AND AGREE TO HOLD HARMLESS OPYF, its officers, coaches, and volunteers.
Does the Participant have any serious injury, illness, allergies or any other medical condition that would prohibit participation in OPYF activities?
From Registration Day until the first game, there will be 50% refund of he original registration fee. After the first game, NO refunds for any reason, nor will player / cheerleader recieve a trophy. For players that quit before the first game, no refund will be made until all equipment is returned.
If any parent or participant has a concern or complaint, the following procedures MUST be followed.
1. Wait 24-hours after the incident (cooler heads will prevail)
2. Talk to the head coach.
3. If the parent is stil not satisfied, the appropriate Vice-President should be contacted.
4. If at this time feels the concerned still has not been corrected, the concern should be brought before the Executive Board for further investigation; and if at this point the parent is still not satisfied, the league Presdent will bring it to the AFY conference board. Not any ant time should any individual contact the AYF Conference or National Board.
1. I, the parent/guardian agree to be financially responsible for the OPYF uniforms and equipment issued on loan to applicant other than normal wear during practices and games. I will reimburse OPYF for loss and.or damages of said uniforms and equipment.
2. I, the parent/guardian, understand that if my child's team/squad qualifies for post season activities including but not limited to National football and cheer events, I will be responsible for all travel, rom, and board expenses. Additional fundraising maybe availiable to minimize these expenses.
3. I, the parent/guardian understand that my child's specific level of participation will be determined by the OPYF board, based on current AYF guidelines (currently 8 plays a game), childs safety and his/her best interest.
4. I, the parent/guardian agree to attend the Mandatory Parent Meeting and abide by the OPYF Parents Code of Ethics and all other OPYF policies.
5. I, the parent/guardian, understand that all paperwork including but limited to, regidtrationform, player/spectator contract, zero tolarence letter, and medical release form before the child can participate in activities.
6. I, the parent/guardian, I agree to allow my childs picture to appear on all OPYF and AYF websites.
7. I, the parent/guardian, understand that I will be required to work AT LEAST one home game in some capacity.
8. I, the parent/guardian, agree to pay all outstanding registration fees before the first game for my child's continued eligibility.
9. I have reviewed a copy of the Oriole Park Player / Spectator Contracts and Zero Tolerance letter. I understand if above participant or either parent violates any of these guidelines, it could lead to family expulsion from the program without refund.
